Periodontics is a dental specialty that involves the prevention, diagnosis, and treatment of diseases of the supporting and surrounding tissues of the teeth or their substitutes and the maintenance of the health, function, and aesthetics of these structures and tissue.
Periodontal disease, like a tooth decay, is an endemic. Almost 100% of the population is affected, while 28% is affected heavily. Every third tooth loss is caused by periodontal inflammation or it may accelerate the development of diabetes, cardiovascular diseases, or a miscarriage.
Periodontics
The supporting structure of the tooth, known as the periodontium, consists of the gums, bones, ligaments, and the healthy relationship between the root. Its health is determined by genetics, which we inherit, as well as by our oral hygiene habits, ranging from daily oral care routines and regular dental check-ups to professional dental cleanings, which should be performed by a dental hygienist at least once a year. However, if this balance is disrupted, inflammation occurs, starting with gingivitis, which is characterized by swelling, a livid red color, and bleeding when pressure is applied, such as during brushing. Untreated gingivitis can progress, leading to loss of attachment of the tight gums, the formation of pockets, gum and bone recession, tooth loosening, and eventually tooth loss.
In advanced periodontal disease, regular annual teeth cleaning is no longer sufficient. The problem is deeper, and therefore the solution is more complex. The formed pockets become progressively deeper, making them more difficult to clean, which allows increasingly aggressive bacteria to proliferate in the oxygen-deprived environment beneath the gums. One possible form of treatment is curettage, which has both closed and open variations.
In closed curettage, no incision is made in the gums. The disadvantage, however, is that we do not have good visibility of the area we are working on. This method is always used in the initial phase. After the oral examination and teeth cleaning, we schedule four consecutive days with the patient. The treatment is always performed under local anesthesia, which is why four appointments are necessary—fewer sessions would numb too large an area at once. In open curettage, we open the gums, allowing us to clean the area with greater effectiveness. This method is not used for severely inflamed gums!
Regular check-ups and teeth cleanings can prevent them.
